Just play one and do what the medal's description tells you to do (although there can be secret medals or the description may need a little deciphering before you figure out what to do). And sometimes, depending on a number of things ranging from your connection speed to Newgrounds server difficulties they might not even register to your profile. They don't really do much anyway, they just sit there on your userpage, slowly accumulating.
